Instructions for Completing Forms

1. Click on first field of form to activate cursor.
2. Fill in each field. Move from field to field using either TAB (forward) or SHIFT-TAB (backward).
3. Click submit button to send form. Most browsers will indicate transmission progress.
4. Click back button on browser to exit page.
